CYSA Forms

• 1601 Youth Membership Form
  For all players new to CYSA. The name should be entered exactly as it appears on the accompanying proof of birth. Print this form and obtain the two (2) required wet-ink signatures: #1 is found in the IMPORTANT INFORMATION box; #2 is found in the CONSENT FOR MEDICAL TREATMENT (MINOR) section. This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over. Players who are age 18 or over MUST sign their own form. The wet ink signatures should be in a color. Two copies of this form must be turned in to your League/Club Registrar with the appropriate fees. You may make a copy of the signed form and turn the original and the copy in or you may turn in two signed copies. Contact your League/Club Registrar if you have any questions. Should accompany 2 - 1x1 in. photo and copy of proof of birth.
• 1606 Team Roster Change
  To add, release or transfer players from your Roster.
• 1611 Single Event Guest Player Form
  For guest players for CYSA sanctioned tournaments if allowed.
• 1627 Foreign Document Translation Form
  If the proof of birth is in a foreign language, print this form and have it translated and approved.
• 1628 Adult Registration
  For all new coaches and staff. Ensure that all appropriate boxes in the "Important Certification" area have been checked. This should accompany 1x1 in photo, and a copy of coaching license (if coaching) and drivers' license.
• Game Card Line Up Stickers
  Fill in the game line ups and print on Avery 5168 paper and use to affix to the game cards that are used for games. Home teams provide game cards to the referees, which are provided by the club registrar.

USCS Forms

• #R002: Player Registration & Medical Treatment Authorization Form
  This form combines the player's medical treatment authorization, and a confirmation the player is registering with the club. This form and proof of birthdate is the only paper information that is required. Keep the original of this form for your records. While this form is still required, and teams must certify they are in possession of it when cards are requested, the team will hold this form unless requested by US Club Soccer.
• #R006: Player Release Form
  Use this form for a mid registration year player release, in addition to submitting a release request on the the website.
• #R007: Player Loan Form
  Use this form to loan a player for a specific competition from one US Club Soccer member to the other. A copy of the completed loan form should be taken by the coach or manager to the competition. The player shall use the player pass issued to his primary club.
• #C006: Referee Report
  Use this referee report as your game report for all US Club Soccer competitions.
• #C007: Competition Incident Report
  Use this form to report any competition incidents which must be presented to the Discipline Committee, pursuant to the US Club Soccer Disciplinary Policies.
